Job Summary
Provide comprehensive care to seniors following clinical standards within a collaborative, multidisciplinary team. Manage referrals, hospital/SNF coordination, DME, and home health services; document care with support from quality-based coders; participate in on-call rotation; contribute to value-based care initiatives. On-site at Conviva Southeast St. Lucie Center in Port Saint Lucie, FL, reporting to Associate Medical Director. Targeted responsibilities include coordinating with care team to deliver high-quality, personalized care and ensuring adherence to Medicare/HEDIS guidelines for senior primary care.
Required Qualifications
- Master's Degree in Nursing or PA program with board certification
- Active, unrestricted NP or PA license in the state of practice
- Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicare/Medicaid numbers
Desired Qualifications
