Job Summary
Coordinate with the Medical Director, attending physicians, and nursing staff to develop direct treatment plans for sub-acute, rehabilitation, and long-term care residents. Perform physical examinations, comprehensive assessments, medical diagnoses, and prescribe treatment modalities while ensuring timely initiation and evaluation of tests, therapies, and medications. Monitor adherence to nursing home policies and standards of care, auditing documentation for regulatory compliance. Provide education to residents, families, and staff, and participate in performance improvement initiatives while respecting cultural and religious diversities. Requires a Master's in NP, NYS license, board certification, and BLS/ACLS; salary range $144,664 to $187,200 annually.
Required Qualifications
- Must have Long-term care experience/Nursing Home experience
- Licensed by the New York State Department of Health
- Certified by the Medicare and Medicaid Program
- Operated by a New York State Licensed Nursing Home Administrator
- Graduate of a Masters in NP program
- Nurse Practitioner License
- NP Board Certification
- BLS and ACLS certification

- Prior experience in acute, sub-acute, rehabilitation and long term care preferred
