Job Summary
Nurse Practitioner required to provide comprehensive primary and geriatric care to medically complex older adults within the PACE model, collaborating with an Interdisciplinary Team to implement and update comprehensive participant Care Plans, perform history and physicals, ensure regular reassessments, coordinate care across settings, support aging in place, and maintain high-quality, person-centered care across PACE clinics, participant homes, hospitals, and facilities; travel to participant homes and facilities as needed, with expected average of five participants per day and participation in on-call rotations; requires Louisiana APRN license, CDS license, DEA registration, and BLS; preferred experience in geriatric care, long-term care, skilled nursing facilities, home health, and PACE programs; strong collaboration and communication skills are essential.
Required Qualifications
- Nursing Degree from an accredited college or university
- Graduate of an accredited Nurse Practitioner Program
- Current and unrestricted Louisiana Advanced Practice Registered Nurse (APRN) license
- Current Louisiana Controlled Dangerous Substance (CDS) license
- Current DEA registration with prescriptive authority
- Current BLS certification
