Nurse Practitioner

Conduct comprehensive in-home, telemedicine, and mobile health/annual wellness visits with Medicare patients as part of their health insurance benefits. Perform face-to-face assessments where members reside or at PCP clinics, reinforcing preventive service adherence and identifying care gaps linked to unnecessary hospitalizations or ER visits. Design your own schedule including weekend visits, with full-time or part-time year-round opportunities. State licensed and Board Certified Nurse Practitioner with NPI required; at least six months of wellness exam experience preferred. Credentialing, training, liability insurance, and mileage reimbursement provided. Ipad, laptop, and web tracking tools supplied to maximize travel time.
