Job Summary
Conduct patient assessments and cardiac evaluations for cardiovascular conditions including CHF, CAD, arrhythmias, and post-MI care. Manage chronic cardiac conditions while assisting with medication management, lifestyle counseling, and patient education. Collaborate with cardiologists to monitor treatment plans and support post-discharge follow-up and transitional care. This daytime role at St. Mary of Nazareth Hospital offers limited weekend or call responsibilities within a high-acuity setting. Required Qualifications
- Current Nurse Practitioner license in Illinois
- Certification as an Adult-Gerontology Acute Care Nurse Practitioner (AGACNP) or Family Nurse Practitioner (FNP) with cardiology experience
- ACLS and BLS certification required
Desired Qualifications
- At least 1 year of experience in cardiology or internal medicine preferred
