Job Summary
Family Nurse Practitioner provides comprehensive primary care to students aged 3-21 in a school-based health center at CAHC, TCAPS West Middle School. Responsibilities include physical assessments, diagnoses, treatment planning, prescribing therapies, preventive health screenings, referrals, care coordination with families and school personnel, accurate documentation compliant with HIPAA/FERPA, participation in interdisciplinary care planning, quality improvement initiatives, health education and outreach, and on-call/after-hours support. Requires MSN, Michigan RN license, FNP certification, Michigan APRN license, BLS, DEA eligibility, credentialing readiness, and valid driver's license. Part-time role based in Traverse City, MI, embedded within a school setting with a focus on pediatric/adolescent primary care and centered on accessible care for students. Required Qualifications
- Master's Degree in Nursing (MSN) from an accredited program
- Active Registered Nurse (RN) license in the State of Michigan
- Active Family Nurse Practitioner (FNP) certification from an accredited national certifying body
- Active Advanced Practice Registered Nurse (APRN) license in the State of Michigan
- Current Basic Life Support (BLS) certification
- Active DEA registration or eligibility to obtain
- Ability to meet credentialing requirements for insurance and regulatory entities
- Valid driver's license
