Job Summary
Perform whole-person clinical assessments by gathering information from external providers, conducting thorough histories and physicals, and ordering additional studies to identify undiagnosed conditions. Lead the medical component of team-based care for individuals with serious mental illness, including training Guides on clinical issues, coordinating with CMHCs and hospitals, and performing post-discharge medication reconciliation. Conduct field visits in patients' homes as needed and make individualized referrals to support disability applications and remediate care gaps. This full-time, on-site role in Richmond, VA offers a 40-hour week with occasional weekend consultative calls, alongside a $30K incentive at 36 months and a $2K continuing education allowance.
Required Qualifications
- Master's degree or higher
- 3+ years of experience in ANY of the following: Primary Care and/or Value Based Care
- Valid Nurse Practitioner (NP) license in the state of Virginia (VA)
- Must be okay visiting patients in their homes, since Home visits are a major component of this position
- Driver's license, private auto insurance, and be insurable under firsthand coverage

- Ability to maintain professional boundaries and engagement skills with a population with challenges and in non-traditional work conditions
- Intermediate computer skills, familiarity with Microsoft Office Suite
