Job Summary
Provide medical care, palliate and manage terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician to adults and children; complete history and physical examinations, order and interpret lab/diagnostic results, diagnose health conditions, and document findings; manage health conditions and hospice care by developing individualized plans, prescribing medications, obtaining consultations or referrals, and coordinating hospice services; complete hospice face-to-face visit requirements for eligibility and recertification; provide clinical information to the hospice medical director; recommend and review plans of care with the interdisciplinary team; develop protocols for treatment; collaborate with the team to revise patient plans of care; fulfill assignments; provide communication and counseling to patients/families on end-of-life issues; perform all other duties as assigned.
Required Qualifications
- Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program
- Must have current nursing licensure in State
- Must have a master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience
- Must have accreditation from an approved certifying body for advanced practice nursing as required by the State
- Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics)
- Must be a licensed driver with licensed automobile that is insured in accordance with organization/state requirements and is in good working order
