Job Summary
Provide hospice medical care as a Nurse Practitioner within an interdisciplinary team, managing terminal illness, performing histories and physicals, developing individualized care plans, prescribing medications, coordinating hospice services, ensuring hospice eligibility assessments, and advising the hospice medical director. Requires NP degree, state license, and at least 1 year of NP and hospice/palliative experience; certification in a specialty (e.g., CHPN) preferred; valid driver and automobile. Positions in Lakewood, CA (Bristol Hospice Greater LA) with in-person care responsibility. Equal Opportunity Employer; E-verify.
Required Qualifications
- Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program
- Must have current nursing licensure in State
- Must have a master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience
- Must have accreditation from an approved certifying body for advanced practice nursing as required by the State
- Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics)
- Must be a licensed driver with licensed automobile that is insured in accordance with organization/state requirements
