Nurse Practitioner

Nurse Practitioner for Bristol Hospice sought to provide medical care and palliate/manage terminal illness under a collaborating physician. Responsibilities include performing history and physical exams, developing individualized care plans, prescribing medications and treatments, coordinating hospice care services, ensuring hospice eligibility per conditions of participation, interfacing with the hospice medical director, reviewing and coordinating patient care with the interdisciplinary team, developing and updating treatment protocols, and counseling patients and families on end-of-life issues. Requires NP master’s degree with relevant hospice/palliative experience, state NP licensure, and accreditation by an approved certifying body; CHPN certification preferred.
