Job Summary
Nurse Practitioner for Bristol Hospice sought to provide medical care and palliate/manage terminal illness under a collaborating physician. Responsibilities include performing history and physical exams, developing individualized care plans, prescribing medications and treatments, coordinating hospice care services, ensuring hospice eligibility per conditions of participation, interfacing with the hospice medical director, reviewing and coordinating patient care with the interdisciplinary team, developing and updating treatment protocols, and counseling patients and families on end-of-life issues. Requires NP master’s degree with relevant hospice/palliative experience, state NP licensure, and accreditation by an approved certifying body; CHPN certification preferred. Required Qualifications
- Must be a graduate of an accredited School of Nursing and accredited Nurse Practitioner Program
- Must have current nursing licensure in State
- Must have a master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience
- Must have accreditation from an approved certifying body for advanced practice nursing as required by the State
- Certification in a specialist area preferred; (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics)
- Must be a licensed driver with licensed automobile that is insured in accordance with organization/state requirements and is in good working order
