Job Summary
Provide medical care, palliate, and manage terminal illness in compliance with the state Nurse Practice Act under a collaborating physician. Complete history and physical examinations, order diagnostic tests, diagnose conditions, and document findings. Develop individualized plans of care, prescribe medications, coordinate hospice services, and fulfill face-to-face visit requirements for eligibility assessment. Collaborate with the interdisciplinary team to review protocols, revise patient care plans, and control symptoms. Consult the physician when plans deviate from standardized practice and counsel patients and families on end-of-life issues. Fulfill assigned assignments and demonstrate knowledge in communication.
Required Qualifications
- graduate of an accredited School of Nursing and accredited Nurse Practitioner Program
- current nursing licensure in State
- master's degree
- minimum of one (1) year Nurse Practitioner experience
- minimum of one (1) year hospice or palliative care experience
- accreditation from an approved certifying body for advanced practice nursing as required by the State
- licensed driver
- licensed automobile that is insured in accordance with organization/state requirements and is in good working order
Desired Qualifications
- Certification in a specialist area (e.g. Hospice and Palliative Nursing (CHPN), Pediatrics or Geriatrics)
