Job Summary
Nurse Practitioner (NP) at Brio Living Services — provide comprehensive assessments, diagnoses, and treatment plans for older adults within Huron Valley PACE; collaborate with physicians, nurses, and care teams; coordinate patient-centered care; perform health histories, exams, lab follow-ups, and documentation; initiate and adjust medications and diagnostics under physician-approved protocols; deliver education to participants and families; participate in quality improvement and regulatory activities; ensure compliance with Michigan scope-of-practice requirements.
Required Qualifications
- RN licensure in the State of Michigan
- certification from a Nurse Practitioner Program
- Bachelor’s Degree in Nursing from an accredited college or university
- Master’s Degree in Clinical Nursing and 5 years nursing experience OR 5–7 years of nursing experience with a minimum of 3 years as a Nurse Practitioner
- Experience in a PACE facility and managed care with geriatric patients
- Proof of clearance for all communicable diseases prior to participant contact
