Job Summary
Nurse Practitioner – Hospice Care providing direct patient care across settings (patients' homes, hospice care centers, and nursing facilities) within an interdisciplinary team. Develops and evaluates individualized care plans and medications in alignment with physician orders and regulatory/organizational policies. Works closely with BCN physicians and community providers to deliver end-of-life care per palliative protocols; responsibilities include advanced nursing practice, patient-family support, and ensuring high-quality, compassionate care in the hospice setting. Schedule is Monday through Friday 8:00am–5:00pm with occasional after-hours and weekend on-call; requires CPR certification, valid driver’s license with a reliable vehicle, and adherence to hospice philosophy.
Required Qualifications
- Master's degree in nursing
- APRN certification as a Family or Adult Nurse Practitioner
- Current APRN licensure in good standing with the State Board of Nursing
- Minimum of three (3) years of general nursing experience; hospice/home health/medical-surgical/critical care experience preferred
- Strong nursing practice skills including pain management; IV skills preferred
- Knowledge of specialty area and ability to translate goals into practice
- Commitment to evidence-based practice and nursing research
- Strong organizational, communication, collaboration, and conflict resolution skills
- Knowledge of healthcare systems and change management processes
- Ability to handle emotional stress and complex patient/family situations with professionalism and compassion
- Complies with accepted professional nursing standards and principles
- Understands and supports hospice care philosophy
- Provides timely, accurate, and detailed documentation; demonstrates strong nursing and medical judgment
- Possesses and maintains CPR certification
- Self-directed, flexible, and able to work with minimal supervision within an interdisciplinary team
- Must be a licensed driver with a vehicle in good working order and insured
