Job Summary
Nurse Practitioner, bilingual, virtual role providing urgent health care via telephone and tele-video for home- and community-based services, with collaboration across providers to develop complex care plans for adults with chronic/complex conditions. Responsibilities include gathering histories, conducting exams as needed, creating and implementing treatment plans aligned with patient goals, identifying gaps in care, coordinating with interdisciplinary teams, educating patients and families on medications, adherence, and crisis prevention, maintaining medical records per state regulations, and participating in required continuing education. Requires MS in Nursing, active NP certification, NP license in TX/IA/KS, RN Compact license, 2+ years of experience in complex care management, EMR proficiency, and bilingual abilities (Spanish preferred). Is offered with a $5,000 sign-on bonus, virtual work with required in-person training sessions, and a hybrid/remote work policy with occasional onsite requirements.
Required Qualifications
- MS in Nursing
- Active national NP certification
- Active NP license (Family or Adult) in TX, IA, or KS
- RN Compact license (valid, current, active)
- 2+ years of experience managing complex care cases
- Experience with Electronic Medical Records (EMR)
- Bilingual or Multi-language skills (Spanish preferred)
