Job Summary
Nurse Practitioner needed to conduct in-home health assessments and manage care for hospice patients in Boise, ID. Responsibilities include performing comprehensive physical exams, vital signs, and symptom reviews; identifying diagnoses and developing care plans; communicating findings to PCPs; educating patients and families on disease processes, medications, and adherence; ensuring HIPAA compliance; and traveling to patient sites for in-home visits. Requires autonomy in the field, ability to navigate varied home environments, and capacity to lift 30 pounds. NP certification (Adult/Geriatric/Family) and Idaho licensure required, with ability to obtain DEA prescriptive authority post-hire. Prior home care or hospice experience and Internal Medicine experience preferred.
Required Qualifications
- Active, unencumbered license to practice nursing in Idaho
- Nurse Practitioner Certification from ANCC, AANP or AACN as Adult, Geriatric or Family
- 1+ year of clinical experience in highest level of education OR 3+ years of experience as a Registered Nurse
- Access to reliable transportation that will enable travel to client and/or patient sites within a designated area
- Ability to obtain DEA licensure/prescriptive authority post-hire in states where applicable
- Ability to spend at least 1 hour with a member in their home, which may be in understaffed or remote areas, in the presence of pets or individuals who are tobacco users
- Ability to lift a 30 pound bag in and out of car and to navigate stairs and a variety of dwelling conditions and configurations
- Experience in Internal Medicine
- Home care or hospice experience
- Excellent administrative and organizational skills and the ability to effectively communicate with seniors and their families
