Job Summary
Ambulatory Program Nurse Practitioner provides comprehensive care for ambulatory patients across programs including the unattached patient clinic, Internal Medicine clinics, Medical Day Unit, and Systemic Treatment Program; collaborates with the interdisciplinary team to ensure seamless transitions of care and leads quality improvements within the Ambulatory Program. Requires registration as a Nurse Practitioner in Ontario (Adult or Primary Health Care), a Master of Science in Nursing (or related masters), and at least 3 years of nursing experience in outpatient/acute care settings with expertise in chronic disease management; demonstrates strong critical thinking, problem-solving, and independent as well as team-based work abilities; must meet physical demands and be fully vaccinated. This is a temporary full-time role within Brockville General Hospital’s Ambulatory Program." }{
Required Qualifications
- Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class with one of the following specialty classes: Adult or Primary Health Care
- Master of Science in Nursing or Masters in a relevant discipline
- Experience working within an outpatient setting
- Experience working within an acute care environment
- Experience with chronic disease management
- Membership with a professional organization such as NPAO, RNAO, or CNA
- Minimum of 3 years nursing experience in respective area required
- Demonstrated ability to deal with and resolve conflict
- Strong critical thinking and problem solving skills
- Demonstrated ability to work independently and as a collaborative member of a team
- Must be able to meet the physical demands of the position
- Double vaccinated with a Canadian recognized COVID vaccine
- Excellent attendance and availability required
