Job Summary
Evaluate, diagnose, and manage acute, chronic, and episodic cardiovascular conditions for patients in Skilled Nursing, Long-Term Care, and Post-Acute Care facilities across the Columbus, OH area. Develop individualized treatment plans, order and interpret diagnostic testing, and prescribe medications while collaborating with physicians and interdisciplinary teams. Round daily at assigned facilities to provide specialty Cardiology care, educate patients and caregivers, and maintain compliant clinical documentation. Full-time daytime schedule with no weekends or on-call duties.
Required Qualifications
- Current, unrestricted Nurse Practitioner license in the applicable state(s) of practice.
- Current national certification appropriate to licensure.
- Current Basic Life Support (BLS) certification.
- Minimum six (6) months of Nurse Practitioner or Physician Assistant clinical experience.
- Valid driver's license with reliable transportation.
- Ability to travel daily throughout an assigned territory.
- Strong clinical judgment, communication, organizational, and critical thinking skills.
- Experience utilizing Electronic Medical Records (EMR).
Desired Qualifications
- Two (2) or more years of Nurse Practitioner experience.
- Experience in Skilled Nursing, Long-Term Care, Post-Acute Care, Internal Medicine, Cardiology, Pulmonology, Hospital Medicine, or Geriatrics.
- Experience caring for medically complex patient populations.
