Job Summary
Provide primary and urgent care services to nursing center residents in collaboration with center leadership and the medical director. Work directly with the Director of Clinical Practice to achieve center-specific goals and prioritize patient medical care. Deliver high-quality health care and everyday compassion to improve lives while adhering to mission pillars. This 4 days per week position rotates between Autumn Lake Healthcare centers in Frederick and Walkersville, MD. The role supports AlignMed Partners' mission of elevating healthcare outcomes for skilled nursing and senior living facilities through a no-cap incentive compensation program.
Required Qualifications
- 1 year or more of provider experience
- graduate from an accredited School of Nursing program
- licensed as a registered nurse
- certified as a Nurse Practitioner by being a nursing graduate from an accredited Nurse Practitioner Program with a Master's Degree in Nursing and have a concentration and specialty track of Older Adult and/or Family Nurse Practitioner (FNP)
- graduate from a program accredited by the ARC-PA
- take the PA National Certifying Exam (PANCE) to use the PAC designation
- PA state licensure
- NCCPA certification
- APRN with an unrestricted professional license
- DEA, for each state in which they practice
- PA with an unrestricted professional license
- DEA for each state in which they practice
Desired Qualifications
- A minimum of two years experience working with adult or geriatric population
