Job Summary
Conduct primary and urgent care services for nursing center residents while collaborating with center leadership and the medical director to achieve high-quality health care goals. Work directly with the Director of Clinical Practice or Area Clinical Practice Manager to align on center-specific priorities and patient-specific medical care plans. Provide compassionate care to adult and geriatric populations within the Hilltop Center, supporting the mission of improving health outcomes for skilled nursing and senior living facilities.
Required Qualifications
- graduate from an accredited School of Nursing program
- licensed as a registered nurse
- certified as a Nurse Practitioner by being a nursing graduate from an accredited Nurse Practitioner Program
- Master's Degree in Nursing
- concentration and specialty track of Older Adult and/or Family Nurse Practitioner (FNP)
- APRN with an unrestricted professional license
- DEA, for each state in which they practice
Desired Qualifications
- two years experience working with adult or geriatric population
