Job Summary

Provide primary and urgent care services to nursing center residents in collaboration with the Center Leadership Team and with the Medical Director/Attending Physician. Deliver high-quality care in a post-acute/long-term care setting, work within a multidisciplinary team to meet center goals, and leverage clinical leadership to improve health outcomes and resident quality of life. The role involves collaborating on-site with center leadership, performing primary and urgent care duties, and utilizing resources and programs (e.g., UpToDate, CAPC, Nabla) to streamline clinical documentation and care delivery. A Master’s Degree in Nursing with a focus on Older Adult and/or Family Nurse Practitioner is required, with preferred experience in geriatric populations and APRN licensure; supervision and opportunities for advancement are highlighted in the benefits package.

Required Qualifications

  • Graduate from an accredited School of Nursing program
  • Licensed as a registered nurse
  • Certified as a Nurse Practitioner by graduating from an accredited Nurse Practitioner Program with a Master’s Degree in Nursing
  • Concentration and specialty track in Older Adult and/or Family Nurse Practitioner (FNP)
  • Will consider other concentrations/specialty tracks based on experience and business need
  • Minimum of two years experience working with adult or geriatric population is preferred
  • APRNs with an unrestricted professional license and DEA, for each state in which they practice
