Job Summary
Nurse Practitioner position in Akron, OH with hybrid remote telehealth and travel across an assigned geographic area, including dialysis clinics and nephrology practices. Responsibilities include completing Comprehensive Health Evaluations (CHEs) for CKD/ESKD, care management to slow disease progression and reduce hospitalizations, prescriptive authority and diagnostic interpretation in collaboration with nephrologists, and clinical leadership to ensure coordinated care. Schedule Monday–Friday, full-time with no nights, weekends, or on-call; emphasis on autonomy and patient-centered outcomes; competitive pay and comprehensive benefits including CEU/CME reimbursement and license renewals. Requires an NP license in Ohio, DEA eligibility within 90 days, CPR/BLS, valid driver’s license, and 2+ years of NP experience; preferred experience in Value-Based Care, Population Health, Cerner/EHR, and 5+ years as a Family NP or related specialty.
Required Qualifications
- Current, unrestricted NP license in state of practice
- DEA license or eligibility within 90 days
- Current CPR/BLS certification
- Valid driver’s license and insurance in the state
- Intermediate computer skills (MS Word, Excel, Outlook)
- Strong clinical judgment, autonomous decision-making, and expertise in managing complex comorbidities
- Minimum 2 years’ experience as a Nurse Practitioner (NP)
