Job Summary
Nurse Practitioner at Oak Street Health delivering value-based primary care to adults on Medicare in an outpatient setting. Work within a collaborative care team to conduct routine/acute visits, annual wellness visits, and health risk assessments; lead care coordination with other providers; support population health initiatives including evidence-based screenings; educate patients on conditions and care plans; participate in promotional activities; and perform home visits as needed to improve patient outcomes.
Required Qualifications
- Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) or NP Certificate Program graduate
- National certification in Family Nurse Practitioner OR Adult-Gerontology Primary Care Nurse Practitioner OR Adult Nurse Practitioner OR Gerontological Nurse Practitioner
- Active, non-probationary state Nurse Practitioner license
- Active DEA license
- US Work Authorization
