Job Summary
Builds relationships with patients and caregivers to ensure high value outcomes. Delivers geriatric primary care using evidence-based guidelines and a multidisciplinary team approach. Performs focused and comprehensive assessments to develop individualized health care plans, manages acute and chronic conditions timely, follows up on interventions, and documents health status accurately. Collaborates with Lifespark team and communicates with family, health services staff, and subspecialists as needed. Integrates into a team of physicians, APRNs, RNs, LPNs, social workers, home health workers, and aides. May involve on-call work that is paid separately; role supports care across client homes, nursing homes, and community settings with a mix of in-person and telehealth visits.
Required Qualifications
- A current Minnesota APRN and RN license and DEA license
- Board certification through ANCC or AANP
- Geriatrics certified preferred OR strong experience
- Meet threshold criteria/credentials for Credentialing
- Communicates effectively in verbal, written, and electronic methods
- Able to work in non-traditional settings (client homes, nursing homes, and from home)
- Strong knowledge of medical management and use of evidence when it exists
