Job Summary
Conduct office visits for routine and acute issues, administering annual wellness visits and health risk assessments with holistic documentation. Provide longitudinal primary care to Medicare adults, collaborating with physicians and care teams while coordinating testing facilities and specialists. Manage population health leadership, ensuring evidence-based screenings for eligible females, and assist with phone triage and patient outreach. Educate patients on care plans and treatments, and conduct home visits as needed. This role reports to the Center Medical Director within our outpatient collaborative care team focused on value-based primary care.
Required Qualifications
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
- National certification in at least one of the following specialties: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, Gerontological Nurse Practitioner
- Active, non-probationary state Nurse Practitioner license
- Active DEA license
- US Work Authorization
Desired Qualifications
- Experience in primary care: internal medicine, geriatrics, or family medicine
- Understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs)
- Passion for teamwork and the opportunity to collaborate cross-functionally
- Desires to be a part of an innovative model focused on empirically-guided population health
- Bilingual proficiency in applicable areas
