Job Summary
Provide effective, value-based primary care to adults on Medicare in an outpatient clinic setting. Conduct office visits for routine and acute issues, perform annual wellness visits and health risk assessments with comprehensive documentation, coordinate care with a multidisciplinary team and external facilities, lead population health efforts (e.g., ensure eligible patients receive evidence-based screenings), assist with triage and outreach by telephone, educate patients about conditions and treatments, participate in promotional activities, and conduct home visits as needed. Collaborate with physicians, physician assistants, nurses, medical scribes, and care managers to deliver high-quality care and support from a network of specialists and hospitals.
Required Qualifications
- Master of Science in Nursing
- Doctor of Nursing Practice
- NP Certificate Program graduate
- National certification in Family Nurse Practitioner OR Adult-Gerontology Primary Care Nurse Practitioner OR Adult Nurse Practitioner OR Gerontological Nurse Practitioner
- Active, non-probationary state Nurse Practitioner license
- Active DEA license
- US Work Authorization
