Nurse Practitioner, Advanced Practice Provider

Provide effective and equitable value-based primary care to adults on Medicare to keep them happy, healthy, and out of the hospital. See patients independently and collaborate with physicians as needed. Practice in outpatient settings on a collaborative care team, including physicians, nurse practitioners or physician assistants, medical assistants, scribes, and care managers. Responsibilities include conducting office visits for routine and acute issues, annual wellness visits and HRAs, care coordination with other providers and facilities, population health leadership within the care team, phone triage and outreach, patient education on health conditions and care plans, participating in promotional activities, and conducting home visits as needed. Reports to the Center Medical Director and works with operational leadership.
