Job Summary
Nurse Practitioner at Oak Street Health providing effective and equitable value-based primary care to adults on Medicare in an outpatient setting. NP sees patients independently within a collaborative care team (physician, NP/PA, medical assistants, scribe, RNs, and care managers), conducts office visits for routine and acute issues, administers annual wellness visits and health risk assessments, coordinates care with specialists and testing facilities, leads population health initiatives, conducts phone triage and outreach, educates patients on conditions and treatment plans, participates in promotional activities, and may conduct home visits as needed. Requires MS in Nursing, DNP, or NP certificate and active NP licensure with valid DEA and US work authorization; certification in one of several NP specialties is required; 40-hour work week with comprehensive benefits and competitive pay.
Required Qualifications
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
- National certification in at least one of the following specialties: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, Gerontological Nurse Practitioner
- Active, non-probationary state Nurse Practitioner license
- Active DEA license
- US Work Authorization
