Job Summary
PHP Nurse Practitioner (Adult and/or Gerontology) at Northwestern Healthcare Center in Berea, OH, provides high-quality primary care in a patient-centered model across settings such as skilled nursing, assisted living, independent living, and telehealth. Supports care through collaboration with PCPs and interdisciplinary teams, conducting wellness visits, diagnosing and managing acute and chronic conditions, ordering/tests, documenting in EMR, and engaging residents and families in care planning. Roles include weekday first shifts, no on-call or weekend/holiday coverage, frequent in-facility presence to ensure face-to-face resident coverage, and a focus on reducing hospital ER visits and maintaining high customer satisfaction. Requires MS in Nursing or Doctor of Nursing Practice or NP certificate, national certification (AANP or ANCC), active unrestricted license, active DEA license, US work authorization, and 2+ years geriatric/adult/family NP experience; strong communication, organization, and teamwork skills; openness to value-based, metrics-driven care and ongoing professional development.
Required Qualifications
- Master of Science in Nursing
- Doctor of Nursing Practice
- NP Certificate Program graduate
- National certification of AANP or ANCC
- Active, unrestricted state license
- Active DEA license
- US work authorization
- 2+ years clinical experience as a Nurse or Nurse Practitioner in geriatric, adult or family practice setting
- Ability to work in a flexible environment and schedule
- Passion for providing healthcare to the senior and low-income populations
