Job Summary

Conduct comprehensive assessments, develop therapeutic plans of care, and provide primary medical services to Redwood Coast PACE participants. Perform home visits, administer injections and immunizations, interpret diagnostic tests, and prescribe medications for episodic and chronic illness management. Coordinate transitions with previous providers, refer to specialists, and actively participate in Interdisciplinary Team meetings and clinical conferences. Monitor participant health, document records per standards, and provide on-call coverage after hours and weekends. Supervise student interns and volunteers while collaborating with the Medical Director and Registered Nurses to deliver geriatric care.

Required Qualifications

  • Graduate of an accredited nurse practitioner program
  • Five (5) years of primary care experience
  • At least three (3) years of experience with a frail or elderly population
  • Registered Nurse and Nurse Practitioner currently licensed by the California Board of Registered Nursing
  • Current Drug Enforcement Administration (DEA) Registration Number
  • Current BLS/ACLS certification within six (6) months of hire
  • Valid California Driver's License
  • Personal auto liability insurance
  • Personal vehicle in good driving/operating condition
  • Clearance of a DOJ and FBI criminal history background check
  • Pre-employment physical
  • PPD skin test
  • Motor Vehicle Report verification
  • Medical knowledge and skills necessary to treat participants and manage complex medical situations
  • Ability to comply with government regulations pertaining to patient care and to community clinics or Redwood Coast PACE
  • Knowledge and skills for the provision of direct participant care in the practice of primary health care
  • Ability to work independently and within an Interdisciplinary Team
  • Sound verbal and written communication skills
  • Ability to handle complex interpersonal and clinical situations
  • Strong administrative and organizational skills
  • Ability to complete duties within an agreed upon time frame
  • Ability to adjust personal schedule as required
  • Basic knowledge of computers and web-based applications such as timekeeping, training, and email programs
  • Physical abilities sufficient to move between different staff work areas
  • Ability to operate a computer
  • Ability to produce reports
  • Ability to talk on the telephone
  • Ability to travel to other agency worksites

Desired Qualifications

  • Experience working as part of an Interdisciplinary Team
  • Valid California Driver's License and personal auto liability insurance preferred
  • Personal vehicle in good driving/operating condition, insured for State minimum liability requirements that can be used for the applicable job functions noted above
  • COVID-19 and Influenza (flu) vaccines preferred
