Job Summary
Conduct daily inpatient medical rounds in a Skilled Nursing Facility, evaluating, diagnosing, and managing acute and chronic conditions while developing individualized treatment plans. Coordinate care with physicians, nursing staff, therapists, specialists, and family members, and complete timely documentation within the EMR. Participate in quality improvement initiatives to ensure compliance with facility and regulatory standards, providing compassionate care that promotes reduced hospital readmissions. This part-time role offers daytime hours with no evening clinic responsibilities, supported by a collaborative physician-led team and an established patient panel.
Required Qualifications
- Current Nurse Practitioner license in the state of practice
- National Board Certification (AANP or ANCC)
- DEA license (or ability to obtain)
