Job Summary
Conduct comprehensive health risk assessments and manage chronic disease conditions for adult and geriatric Members across skilled nursing and long-term care facilities. Collaborate with physicians, case managers, and interdisciplinary teams to coordinate care, interpret diagnostic data, and create evidence-based treatment plans including medication reconciliation and wellness counseling. Independently travel between assigned Texas facilities to deliver provider-based care, maintain team schedules, and support clinical leadership in post-acute workflows. Achieve quality improvement targets while ensuring HIPAA compliance and addressing unmet needs through appropriate referrals.
Required Qualifications
- Active RN and NP licensure in the state of practice
- Board certification (AANP or ANCC)
- Current CPR certification
- Valid driver's license
- Automobile insurance
- Ability to independently travel to all assigned locations
- Ability to lift and/or move up to 50 lbs
- Close and peripheral vision, hearing, and ability to adjust focus
- Manual dexterity to operate medical equipment and office technology
- Moderate to intense concentration required due to clinical complexity
Desired Qualifications
- 1 year of experience as a Nurse Practitioner in adult, geriatric, or family practice settings
- Graduate of an accredited NP program (Adult/Gerontology Primary Care preferred)
- Geriatric certification (or willingness to obtain)
- Bilingual (English/Spanish)
- Familiarity with community resources and behavioral health needs
- Experience serving vulnerable, low-income, or disadvantaged populations
- Highly organized, detail-oriented, and committed to ongoing learning and professional development
