Job Summary
Provide clinic-based and home/telehealth care as a Nurse Practitioner in Columbia and Glen Burnie, MD. Develop and implement clinical plans of care for adults with chronic conditions (e.g., diabetes, COPD, CHF, debility, dementia); perform history, physical exams, and diagnostics in home or clinic settings; collaborate with an interdisciplinary team to address physical and psychosocial needs; educate patients and families on medications, diet, adherence, and crisis prevention; communicate with other clinicians to coordinate care and maintain records consistent with state regulations; participate in ongoing education, documentation per risk-adjustment standards, and weekly interdisciplinary meetings; prescribe medications as permitted by the state Board of Nursing; weekday schedule (M-F, 8-5); on-site visits with potential for telehealth and home-based care.
Required Qualifications
- Master’s degree in nursing
- Board Certification as an Adult, Adult-Geriatric or Family Nurse Practitioner
- Current State Registered Nurse & Nurse Practitioner license
- DEA registration or eligibility
- Basic Life Support (BLS) certification
- Minimum of one (1) year NP experience preferred in primary care/internal medicine
- Experience with value-based care preferred
- Proficiency with EMR (AthenaHealth highly preferred)
- Proficiency with Microsoft Office
- Excellent communication skills
