Job Summary
Nurse Practitioner (100% virtual with required in-person training) providing urgent health care via telephone and tele-video to home- and community-based patients enrolled in state Medicaid programs. Collaborates with company physicians and other providers to develop complex plans of care for adult patients with chronic/complex conditions, gathers history and exams, implements treatment plans, closes gaps in care, educates patients and families on medications, adherence, and crisis prevention, and maintains documentation per state regulations. Requires MS in Nursing, active NP certification, TX/IA/KS NP license, RN Compact license, 2+ years of managing complex care cases, and EMR experience. Hybrid/work pattern includes regular remote work with in-person training and occasional on-site requirements; shift is Monday–Friday 8:00am–5:00pm CST with rotating on-call; role supports CareBridge Health within Elevance Health family, with compliance and vaccination policy considerations embedded.
Required Qualifications
- MS in Nursing
- Active national NP certification
- Valid, current, active and unrestricted Family or Adult NP license in TX/IA/KS
- Valid, current, active RN Compact license
- 2+ years of experience in managing complex care cases
- Experience with Electronic Medical Records (EMR)
