Job Summary
Provide preventive, acute, and chronic care while partnering with patients to create personalized care plans. Collaborate with an interdisciplinary care team focused on whole-person health, using evidence-based medicine and population health strategies to improve outcomes. Empower patients through education, coaching, and shared decision-making within a model featuring 10-15 patients daily and longer appointment times. Support is provided by a dedicated Whole Health Coach embedded in the care team.
Required Qualifications
- MSN from an accredited program
- Board Certified Nurse Practitioner
- Current RN/APRN license
- DEA registration
