Job Summary
Assess patients' medical and mental health needs, prescribe medications, order lab work and diagnostic procedures, and modify treatment plans based on patient response. Provide personalized, coordinated health care to long-term care residents enrolled in Medicare Advantage plans, proactively managing acute and chronic conditions to prevent unnecessary hospital visits. Ensure compliance with local, state, and federal regulations while collaborating with RN case managers and facility partners to implement the national model of care. Exercise independent clinical judgment with physician consultation as appropriate.
Required Qualifications
- FNP, AGNP, AHACNP or PA license
- Degree from an accredited APRN or PA program
- 3 years' experience in clinical nursing or rehab in geriatric populations
- Electronic Health Records experience
- Working knowledge of Microsoft applications, including Word, Outlook and Excel
Desired Qualifications
- experience in primary care or internal medicine, preferably with geriatrics focus
