Job Summary
Nurse Post Discharge Coordinator coordinates discharge planning, post-discharge crisis calls, resource connections to aftercare services, and documentation of patient status in the EMR. In collaboration with a multidisciplinary team, the role supports discharge planning and quality assurance measures, follows the Zero Suicide protocol, and, in a hybrid capacity, assists the hospital’s nursing department. Responsibilities include managing discharge reference and post-discharge tracking tools, updating county resource guides, facilitating post-discharge coordination and linkages to appropriate resources, ensuring follow-up appointments with psychiatry and therapy within 7 days of discharge, conducting C-SSRS assessments when suicidality is reported, reviewing safety planning, documenting services in the EMR, responding to voicemails within 24 hours, maintaining professional relationships with internal and external stakeholders, supporting departmental administrative duties and data tracking for QAPI/compliance, training staff on measurable outcomes, and performing other duties as assigned. Must maintain confidentiality, demonstrate strong organizational and communication skills, work independently and as part of a team, and adapt to a fast-paced, growing hospital environment.
Required Qualifications
- Licensed as an RN, LVN, or LPT in the state of California that is active and current
- One year of behavioral health experience and experience with Severely Mentally Ill and discharge planning preferred
- Current CPR (Cardiopulmonary Resuscitation) certification (or obtained within 30 days of hire)
- Current Handle with Care (HWC) certification (or obtained within the 30 days of hire)
- Familiarity with continuum of care and clinical terminology
