Job Summary
Provide day-to-day nursing care and case management under NP supervision in a homeless shelter and community-based setting, including direct client care, outreach, and home visits. Responsibilities include medication management, coordinating care plans, conducting assessments, mentoring nursing students, and working within an interdisciplinary team (NP, RN, LCSW, Peer Recovery Specialist) to support individuals experiencing homelessness and complex health challenges. Skills emphasized include compassion, flexibility, strong communication, and self-motivation, with evenings potentially required and the ability to drive a van for program needs.
Required Qualifications
- Active RN license
- 1–2 years clinical experience
- Valid driver’s license and ability to drive a van
- Ability to lift up to 50 lbs
- Ability to work independently under supervision
