Job Summary
Nurse Navigator for SF Head and Neck Surgery provides individualized assistance to patients and families to help overcome health care system barriers, coordinating care across the continuum, and educating patients to support shared decision making and alignment with patients' values. Works with physicians and the interdisciplinary team to triage, coordinate and manage patient care, facilitates timely access to services, coordinates appointments and tests, and supports survivorship and literacy needs. Requires strong communication, education, and organizational skills; 5 years of relevant experience is typical, with RN licensure in California, BLS, and oncology-focused certifications/or equivalents as part of entity requirements. Proficiency in medical terminology, documentation, care coordination, and knowledge of reimbursement is expected, along with the ability to operate within Sutter Health’s quality and patient-centered care framework.
Required Qualifications
- RN license in California
- BLS - Basic Life Support Healthcare Provider
- Oncology Nursing Certification (ONS) or equivalents (CPHOB, CBCN, BMTCN, APHON or AOCNP) if required by entity
- ONS Foundations of Oncology Nursing Practice (within 90 days if less than 1 year oncology experience)
- Graduate of an accredited school of nursing
