Nurse Navigator Pain Clinic

Nurse Navigator for Pain Clinic coordinates care across the healthcare continuum by analyzing medical records, collaborating with physicians, patients and families, and coordinating referrals, home care services, and DME to develop realistic treatment plans. Serves as a patient advocate, ensures timely communication of test results, and supports standards of care and diagnosis-based protocols. Requires an Associate in Nursing, RN licensure, BCLS, and 1 year of inpatient/outpatient clinical experience (2 years nursing experience required; 3 years in designated specialty preferred). The role supports effective care coordination, education, and resource provision to patients and families, with day shift hours in Scottsdale, AZ within the HonorHealth Pain Care department.
