Nurse Navigator - Oncology

Coordinate the care of patients through the entire cancer care continuum, including diagnosis, treatment, survivorship, and end-of-life care. Evaluate patient needs and facilitate timely access to quality health and psychosocial care by overcoming system barriers. Provide education to patients and caregivers to support appointment compliance and treatment adherence. Collaborate with interdisciplinary teams and external services to ensure seamless coordination of chemotherapy, infusion therapy, radiation, and other treatments. Assist in coordinating tumor board agendas and participate in quality data collection to optimize processes. Respond to emergency situations and adhere to safety policies while promoting a safe working environment. Support patients with new, recurrent, or metastatic cancer diagnoses across medical, surgical, radiation, genetics, and palliative care departments.
