Job Summary
Nurse Navigator (RN) in Oncology Clinic provides disease-specific guidance and resources to patients, families, and caregivers to overcome barriers to cancer care. The role interfaces with patients and providers in person, by phone, and electronically to promote timely access to care, coordinates care using the nursing process, educates patients, and supports program goals. Core duties include acting as subject matter expert in navigation, improving care timeliness and access, collaborating with program leaders, delivering patient-centered care tailored to diverse populations, coordinating care across multidisciplinary teams, documenting and communicating care plans, and serving as a preceptor/mentor for other nurses. Required qualifications include RN licensure with at least Associates/Diploma/Bachelor’s in Nursing (Bachelor’s preferred), BLS certification, and multi-state RN options as applicable; minimum 3 years nursing practice with 2 years in disease-specific care and outpatient care or care coordination preferred; strong time management, critical thinking, and ability to operate in a fast-paced environment.
