Job Summary
Serves as a licensed registered nurse providing individualized assistance to patients, families, and caregivers to overcome barriers to care and facilitate timely access to quality health and psychosocial services throughout the cancer continuum. Utilizes the nursing process to coordinate interdisciplinary care, document safe quality patient care, and bridge resources to address gaps in services. Collaborates with program leaders to evaluate community needs, supports performance improvement initiatives, and acts as a preceptor for other nursing staff. Requires a BLS certification, RN license, and minimum three years of nursing practice with a preference for disease-specific and outpatient care coordination experience. Works a day shift in Augusta, GA, within the Wellstar Specialty Program.
Required Qualifications
- Licensed registered nurse
- Disease-specific clinical knowledge
- Associates Nursing or Diploma (Nurse) Nursing or Bachelors Nursing
- BLS - Basic Life Support or ARC-BLS - Amer Red Cross Basic Life Support or BLS-I - Basic Life Support - Instructor
- RN - Reg Nurse (Single State) or RN-COMPACT - RN - Multi-state Compact
- Minimum 3 years nursing practice
- Ability to function in a fast paced environment
- Can respond to emergencies in using a decisive, composed and respectful manner
- Possesses excellent time management skills
- Possesses strong critical thinking skills

- Bachelors Nursing
- Minimum 2 years nursing with disease specific experience
- Outpatient care or care coordination experience
