Job Summary
Conduct continuity of care by ensuring smooth transitions between settings and managing high-risk, complex patients to minimize readmissions. Facilitate movement along the healthcare continuum in collaboration with the multi-disciplinary team, physicians, and third-party payers to achieve quality, cost-effective outcomes. Discuss alternative care options and assist with discharge planning needs while identifying barriers to care to elicit process improvements. Prepare and execute post-discharge care plans as the patient advocate, adapting communication styles to specific patient populations. Requires an RN license, two years of nursing experience, and BLS certification, with a Monday through Friday schedule from 8am to 5pm.
Required Qualifications
- Registered nurse diploma
- 2 years of nursing experience
- Current registered nurse (RN) license in state of practice
- Basic Life Support (BLS) from the American Heart Association
- Proficiency in using computers, software, and web-based applications
- Effective verbal and written communication skills
- Ability to work a flexible work schedule (e.g. 24/7, weekend, holiday, on call availability)
- Ability to travel throughout and between facilities
- Frequently exerting 10 to 20 pounds of force to move objects
- Occasionally exerting up to 100 pounds of force
- Residence in and work location in Louisiana (not in Colorado, California, Hawaii, Illinois, Maine, Maryland, Massachusetts, Minnesota, New Jersey, New York, Vermont, Washington, or Washington D.C.)
Desired Qualifications
- Associate or bachelor's degree in nursing
- Certification in clinical specialty area
