Nurse Navigator - Inpatient Oncology Transitions

Job Summary

Navigate patients through diagnostic resolution and transition to specialty care as needed. Apply evidence-based guidelines to ensure timely follow-up and support consistent care coordination across the continuum. Improve timely access to specialty oncology care while reducing friction at high-risk transition points. Optimize capacity, scheduling accuracy, and throughput to advance standardization, equity, and patient-centered access. Improve patient and provider satisfaction around oncology access. Work day shift with 40 weekly hours. Requires BSN or MSN/DNP with three years of clinical nursing experience and oncology knowledge.

Required Qualifications

  • Bachelor's degree in Nursing from an accredited school of nursing
  • Minimum of three years related clinical nursing experience
  • Licensure as a registered nurse by the South Carolina Board of Nursing or a compact state
  • Current American Heart Association (AHA) Basic Life Support (BLS) certification
  • Current American Red Cross BLS for Healthcare Providers certification
  • Ability to lift and carry 50 lbs. unassisted
  • Ability to lift/lower objects 50 lbs. from/to floor from/to 36 inches unassisted
  • Ability to lift from 36" to overhead 25 lbs.
  • Ability to exert up to 50 lbs. of force
  • Ability to maintain 20/40 vision, corrected, in one eye or with both eyes
  • Ability to see and recognize objects close at hand or at a distance
  • Ability to match or discriminate between colors
  • Ability to determine distance/relationship between objects; depth perception
  • Good peripheral vision capabilities
  • Ability to maintain hearing acuity, with correction
  • Ability to maintain tactile sensory functions
  • Ability to maintain good olfactory sensory function
  • Ability to be qualified physically for respirator use, initially and as required
  • Ability to work rotating shifts
  • Ability to work overtime as required

Desired Qualifications

  • Oncology nursing experience
  • Master of Nursing (MSN) or Doctor of Nursing Practice (DNP) in lieu of BSN
  • Considerable knowledge of complex medical terminology and advanced disease processes
  • Strong interpersonal skills
  • Ability to delegate to appropriate personnel
  • Ability to establish and maintain collaborative relationships both internally and externally
  • Strong organizational skills
