Job Summary
Nurse Navigator I role focusing on enhancing intake and care planning for subacute clients, coordinating and managing care plans via telehealth or phone, supporting nurse contractors and administrative teams, and escalating to Nurse Navigator II/Nursing Manager as needed. Requires RN license in Arizona, BLS/ACLS, ASN or higher, 4+ years of direct client care with acute experience, strong communication, and comfort with Google Suite and healthcare platforms; 40-hour weekly schedule with remote/hybrid flexibility; initial in-person training at DoN.
Required Qualifications
- Registered Nurse (RN) with Arizona license
- Current AHA BLS and/or ACLS
- ASN or higher & graduated from an accredited nursing program
- 4+ years of direct client care experience, with at least 1 year in an acute setting
- Proficient with Google Suite, healthcare platforms, and adaptable to new technologies
- Complete the 20+ hour in-person training with DoN
