Support patients and families by serving as a patient advocate, educating them on care options, and coordinating multi-disciplinary treatment plans. Secure prescriptions and supplies as needed while participating in telephone triage and scheduling refill appointments. Provide direct care during provider visits and consistently follow evidence-based nursing standards to ensure safe patient outcomes. Work as a key member of the interdisciplinary care team, fostering a culture of respect and collaboration. Requires a current NJ-RN license and BSN degree.
