Job Summary
Coordinate multidisciplinary care for new patients by assessing needs, triaging information, and scheduling appointments across the healthcare team. Collaborate with physicians, family, and ancillary staff to create care plans, educate patients on diagnoses and therapies, and manage transitions in care. Research current evidence and trials to guide practice decisions and serve as a resource for prospective patients. Assess support systems and barriers to learning while ensuring compliance with the Illinois Practice Act and ANA standards. Maintain patient safety through effective communication and critical thinking in clinical decision-making.
Required Qualifications
- Current experience within the past five years
- Completion of an accredited Registered Nurse program with a BSN and RN License in the state of Illinois (LIC RN)
- CERT BLS
- Basic computer knowledge
- Ability to communicate effectively in English, both verbally and in writing
- Excellent customer service skills
- Consistently works within the framework of the Illinois Practice Act for Nursing and ANA Standards of Professional Nursing Practice
- Ability to assess and determine situations requiring an interpreter and to use interpreter services effectively
Desired Qualifications
- One year of experience in acute care in a hospital setting and/or one year of experience in outpatient setting. This experience is preferred to be within the last four years of employment
- Prior case management or patient navigation experience in an acute or outpatient
- Basic EPIC EMR knowledge
