Nurse Navigator-Community

Nurse Navigator for Care Management Orthopedic Navigation (0.8 FTE; 64 hours per two-week pay period) overseeing comprehensive assessment and care coordination to optimize health outcomes. Remote position with occasional required travel to meetings; must reside within commuting distance of Allina Commons, Minneapolis MN. Responsibilities include conducting comprehensive assessments, developing person-centered care plans, coordinating an interdisciplinary care team, providing clinical consultation, facilitating plan implementation and transitions across the continuum of care, using motivational interviewing to engage participants, coordinating services and resources, documenting encounters, and supervising care-management support tasks. Requires RN licensure in MN (and WI within 90 days of hire if applicable), 2–5 years of clinical experience; preferred BSN; may involve travel and collaboration with multiple providers to deliver integrated care. This role supports the new Care Management Orthopedic Navigation program and is part of a broader Allina Health care team focused on quality, cost-effective care and patient-centered outcomes.
