Job Summary
Manage cardiovascular hospitalized patients as a nurse navigator, case manager, and patient/physician liaison to ensure clinical, quality, and cost-effective outcomes. Collect, trend, and analyze aggregate outcomes data to identify best practices and target performance improvement efforts. Apply triage principles and specialized knowledge to interpret policies, procedures, and programs while coordinating with nursing staff, physicians, families, and community agencies. Requires substantial clinical competency, judgment, and self-direction in a physician-led institute serving complex structural heart disease and advanced heart failure cases. Bachelor's degree in Nursing required; MSN, PhD, or DNP preferred.
Required Qualifications
- Bachelor's degree in Nursing
- Current Registered Nurse license in Washington State or Multistate License endorsement (MLS)
- Current BLS Certification (if direct patient contact)
- Three (3) recent clinical experiences in area of specialty
Desired Qualifications
- Advanced degree in Nursing (MSN, PhD, DNP, ARNP)
- Certification in assigned area
