Job Summary
Coordinate and supervise delivery of Community Nursing Health Services by providing coordination, supervision, and coaching to field nurses. Develop and maintain effective Care Plans for Community Health Services, independently or with Ontario Health atHome, while monitoring employee performance against Regulated Health Professions Act policies. Promote Community Health Services through public relations and provide active guidance for workplace Health and Safety. Manage on-call rotation including evenings and weekends, and perform other duties as required. Requires RN degree, CNO registration, and vehicle for travel.
Required Qualifications
- Registered Nurse (RN) degree
- 3 - 5 years of job-related experience or an equivalent combination of education and experience
- Registration with the College of Nurses of Ontario (CNO)
- Proven computer skills, including email, word processing, work scheduling systems
- Thorough knowledge of case management skills and nursing processes and practices
- Vehicle is required
- some regular travel is required
- Clear Background and Vulnerable Sector Check
Desired Qualifications
- Leadership and people management skills are an asset
- Excellent English verbal and written communication skills
- French language skills are an asset
